About Cagrilintide

Cagrilintide is an acylated analogue of human amylin — the hormone co-secreted with insulin from the same pancreatic beta cells, and a completely separate signalling axis from GLP-1.

Native amylin is impractical to work with. It aggregates readily, forming the fibrils that make it difficult to keep in solution at all. Cagrilintide's backbone borrows from calcitonin, a structurally related but non-aggregating peptide, which is what makes a stable long-acting amylin analogue possible where the native sequence is not.

The second modification is a eicosanedioic acid chain attached through a gamma-glutamate spacer. That is the same albumin-binding trick semaglutide uses, and it is why both persist long enough for weekly rather than daily protocols. Two peptides from unrelated hormone families, solving the same half-life problem the same way.

It is stocked here both on its own and co-formulated with semaglutide. The separate vials exist because the interesting question is usually whether amylin and incretin signalling are additive or overlapping, and answering that needs each one variable independently.
