Comparison
Both act at the ghrelin receptor. One is the most selective compound in the GHRP family and the other is the most potent, and those two properties pull against each other.
Ipamorelin is the control compound precisely because it does so little else: minimal cortisol and prolactin involvement means ghrelin receptor activity can be studied in isolation. Hexarelin is more potent at that receptor but also binds cardiac CD36, so it is the right choice only when that second target is what the study is about, and the wrong one when it is a confound.

It produces minimal activity at cortisol and prolactin pathways compared with the other growth hormone releasing peptides. That absence of secondary effects is what makes it the standard control in comparative work.
CD36 is a scavenger receptor expressed in cardiac tissue. Hexarelin binds it in addition to the ghrelin receptor, which gives it a second mechanism the other GHRPs do not have and makes it useful or confounding depending on the research question.
GHRPs such as these act at the ghrelin receptor. GHRH analogues such as CJC-1295 and sermorelin act at the growth hormone releasing hormone receptor. They are the two separate arms of the same axis, which is why blends pairing one of each are common.
